Introduction
Furosemide is a loop diuretic commonly used to treat conditions that involve fluid retention (edema) and hypertension. It works by inhibiting sodium and chloride reabsorption in the loop of Henle in the kidneys, leading to increased urine production and reduced fluid buildup in the body.
Conditions of Use (Indications)
- Edema: Used to treat fluid retention associated with conditions such as congestive heart failure, liver cirrhosis, and nephrotic syndrome.
- Hypertension: Used to treat high blood pressure, often in combination with other antihypertensive medications.
- Acute Pulmonary Edema: Used in emergency situations to reduce fluid buildup in the lungs.
- Chronic Renal Disease: Can be used to manage fluid overload in patients with chronic kidney disease.
Mechanism of Action
Furosemide inhibits the Na+/K+/2Cl- co-transporter in the thick ascending limb of the loop of Henle in the kidneys. This reduces the reabsorption of sodium and chloride, which increases urine output and decreases fluid retention in the body. Furosemide also reduces blood volume and lowers blood pressure.
Side Effects
1.Common:
- Electrolyte imbalances, particularly hypokalemia (low potassium).
- Dehydration and hypotension.
- Dizziness and headache.
2.Serious:
- Ototoxicity (hearing loss or ringing in the ears), especially with high doses.
- Acute renal failure, particularly in patients with preexisting kidney conditions.
- Hyperglycemia, especially in diabetic patients.
Harmful Interactions
- Aminoglycoside Antibiotics: Increased risk of ototoxicity when used with furosemide.
- Lithium: Furosemide may reduce lithium clearance, increasing the risk of lithium toxicity.
- NSAIDs (Nonsteroidal Anti-Inflammatory Drugs): May reduce the diuretic effect of furosemide.
Contraindications
- Known hypersensitivity to furosemide or sulfonamide-derived drugs.
- Anuria (inability to produce urine).
- Severe electrolyte imbalances (e.g., hypokalemia, hyponatremia).
- Severe liver disease with hepatic coma.
