Introduction

Warfarin is an anticoagulant (blood thinner) used to prevent and treat various thromboembolic disorders, including deep vein thrombosis (DVT), pulmonary embolism (PE), and atrial fibrillation (AF) to prevent stroke.

Conditions of Use (Indications)

1. Atrial Fibrillation: Used to prevent stroke in patients with AF by inhibiting blood clot formation.
2. Deep Vein Thrombosis (DVT): To prevent and treat blood clots in the veins.
3. Pulmonary Embolism (PE): For the treatment and prevention of blood clots in the lungs.
4. Mechanical Heart Valves: To prevent thromboembolic events in patients with mechanical heart valves.

Mechanism of Action

Warfarin works by inhibiting the enzyme vitamin K epoxide reductase, which is responsible for recycling vitamin K in the body. Vitamin K is essential for the synthesis of clotting factors II, VII, IX, and X. By inhibiting vitamin K, warfarin decreases the synthesis of these clotting factors, leading to an anticoagulant effect.

Side Effects

1. Common:

• Bleeding complications (bruising, nosebleeds, or gum bleeding).
• Nausea, vomiting, and diarrhea.

2.Serious:

• Major bleeding, including intracranial hemorrhage, which can be life-threatening.
• Skin necrosis (rare) due to warfarin-induced tissue death, particularly in patients with protein C or S deficiency.

Harmful Interactions

1. CYP2C9 Inhibitors: Drugs like fluconazole and amiodarone can increase warfarin levels, increasing the risk of bleeding.
2. NSAIDs: Nonsteroidal anti-inflammatory drugs (e.g., ibuprofen, naproxen) can increase bleeding risk when used with warfarin.
3. Vitamin K-Rich Foods: High consumption of vitamin K-rich foods (e.g., leafy greens) can reduce the effectiveness of warfarin by increasing clotting factor synthesis.

Contraindications

1. Active bleeding disorders (e.g., peptic ulcer, intracranial hemorrhage).
2. Pregnancy (warfarin is teratogenic and can cause fetal harm).
3. Recent major surgery or trauma.
