Introduction

Topiramate is an antiepileptic drug (AED) and a migraine prophylactic agent. It is also used for weight management in combination with other therapies.

Conditions of Use (Indications)

1. Epilepsy:

• Partial-onset seizures, generalized tonic-clonic seizures, Lennox-Gastaut syndrome.

2. Migraine Prophylaxis:

• Reducing the frequency and severity of migraine attacks.

3. Weight Management (Off-Label):

• Used as part of combination therapy in obesity treatment.

Mechanism of Action

Topiramate has multiple mechanisms of action:

1. Blocks voltage-dependent sodium channels, reducing neuronal excitability.
2. Enhances the activity of gamma-aminobutyric acid (GABA) at GABA-A receptors, promoting inhibition.
3. Antagonizes glutamate at AMPA/kainate receptors, reducing excitatory neurotransmission.
4. Inhibits carbonic anhydrase, contributing to its diverse effects.

Side Effects

1. Common Side Effects:

• Paresthesia, fatigue, cognitive slowing, weight loss, dizziness.

2. Serious Side Effects:

• Kidney stones, metabolic acidosis, glaucoma, hyperammonemia, teratogenic effects.

Harmful Interactions

1. Carbonic Anhydrase Inhibitors (e.g., acetazolamide):

• Increased risk of metabolic acidosis and kidney stones.

2.CNS Depressants:

• Additive sedation or cognitive impairment.

3. Oral Contraceptives:

• Reduced efficacy due to increased metabolism.

Contraindications

1. Hypersensitivity to topiramate.
2. Pregnancy (due to risk of cleft palate and other congenital malformations).
3. Metabolic acidosis in patients on metformin (caution required).
