Introduction

Dapagliflozin is a sodium-glucose co-transporter 2 (SGLT2) inhibitor used in the management of type 2 diabetes, heart failure, and chronic kidney disease (CKD). It works by preventing the reabsorption of glucose in the kidneys, leading to increased glucose excretion in the urine and improved glycemic control.

Conditions of Use (Indications)

  1. Type 2 Diabetes: Helps to lower blood sugar levels in patients with type 2 diabetes, especially when diet and exercise alone are insufficient.
  2. Heart Failure: Used to reduce the risk of hospitalization and cardiovascular death in heart failure patients with reduced ejection fraction.
  3. Chronic Kidney Disease: Slows the progression of kidney disease in patients with chronic kidney disease and type 2 diabetes.

Mechanism of Action

Dapagliflozin inhibits the SGLT2 protein in the kidneys, which is responsible for reabsorbing glucose from the urine back into the bloodstream. By blocking this transporter, dapagliflozin promotes the excretion of excess glucose in the urine, leading to lower blood sugar levels and a mild diuretic effect. This mechanism is beneficial in reducing the risk of complications in type 2 diabetes, heart failure, and CKD.

Side Effects

1. Common:

  • Urinary tract infections (UTIs), yeast infections, and increased urination.
  • Dizziness, dehydration, and hypotension.
  • Increased cholesterol levels.

2.Serious:

  • Diabetic ketoacidosis (rare but serious).
  • Acute kidney injury or worsening renal function.
  • Severe hypotension, especially in patients with volume depletion.

Harmful Interactions

  1. Diuretics: Increased risk of dehydration and hypotension.
  2. ACE Inhibitors/Angiotensin Receptor Blockers (ARBs): May exacerbate renal dysfunction when combined with SGLT2 inhibitors.
  3. Insulin or Insulin Secretagogues: Increased risk of hypoglycemia, requiring dosage adjustments.

Contraindications

  1. Hypersensitivity to dapagliflozin or other SGLT2 inhibitors.
  2. Severe renal impairment or end-stage renal disease.
  3. Active bladder cancer (due to a potential increase in cancer risk).
  4. Pregnancy and breastfeeding (use only if benefits outweigh risks).
