Introduction

Ibuprofen is a nonsteroidal anti-inflammatory drug (NSAID) commonly used for pain relief, fever reduction, and inflammation control. It works by inhibiting cyclooxygenase (COX), an enzyme responsible for the production of prostaglandins, which are involved in pain, fever, and inflammation.

Conditions of Use (Indications)

  1. Pain: Used for mild to moderate pain, including headaches, menstrual cramps, dental pain, and musculoskeletal pain.
  2. Fever: Reduces fever in various conditions like colds and infections.
  3. Inflammation: Treats inflammatory conditions such as rheumatoid arthritis, osteoarthritis, and ankylosing spondylitis.
  4. Post-operative Pain: Often used following surgery to manage pain and reduce inflammation.

Mechanism of Action

Ibuprofen inhibits both COX-1 and COX-2 enzymes, which are responsible for producing prostaglandins. COX-1 is involved in maintaining normal cellular function, while COX-2 is more involved in inflammation. By inhibiting COX enzymes, ibuprofen reduces the synthesis of prostaglandins, leading to decreased pain, fever, and inflammation.

Side Effects

1.Common:

  • Gastrointestinal upset, including nausea, dyspepsia, and abdominal pain.
  • Dizziness and headache.
  • Fluid retention and edema.

2.Serious:

  • Gastrointestinal bleeding or ulceration, especially with long-term use.
  • Renal impairment, particularly in patients with pre-existing kidney conditions.
  • Cardiovascular risks, including hypertension and an increased risk of myocardial infarction or stroke with long-term use.

Harmful Interactions

  1. Anticoagulants (e.g., Warfarin): Ibuprofen may increase the risk of bleeding when used with anticoagulants.
  2. Diuretics: NSAIDs like ibuprofen may reduce the effectiveness of diuretics.
  3. ACE Inhibitors: When combined with ACE inhibitors, ibuprofen may lead to acute renal failure, particularly in dehydrated individuals.

Contraindications

  1. Known hypersensitivity to ibuprofen or other NSAIDs.
  2. Active gastrointestinal bleeding or peptic ulcer disease.
  3. Severe liver or renal impairment.
  4. Pregnancy (particularly in the third trimester).
