Introduction

Lorazepam is a benzodiazepine with anxiolytic, sedative, hypnotic, anticonvulsant, and muscle-relaxant properties. It is commonly used for anxiety disorders, insomnia, and seizure management. It has a relatively intermediate duration of action compared to other benzodiazepines.

Conditions of Use (Indications)

1. Anxiety Disorders:

  • Short-term management of severe anxiety.

2. Insomnia:

  • Particularly when associated with anxiety.

3. Seizures and Status Epilepticus:

  • First-line agent for terminating status epilepticus.

4. Preoperative Sedation:

  • Used to induce sedation and reduce anxiety prior to procedures.

5. Alcohol Withdrawal:

  • To prevent or manage withdrawal symptoms, including seizures.

Mechanism of Action

Lorazepam enhances the effect of gamma-aminobutyric acid (GABA), the primary inhibitory neurotransmitter in the brain. It binds to the benzodiazepine site on the GABA-A receptor, increasing chloride ion influx, hyperpolarizing neurons, and reducing excitability.

Side Effects

1. Common Side Effects:

  • Sedation, dizziness, weakness, fatigue.
  • Cognitive impairment, ataxia, and amnesia.

2. Serious Side Effects:

  • Respiratory depression, especially when combined with other CNS depressants.
  • Dependence, tolerance, and withdrawal syndrome with long-term use.

Harmful Interactions

1. CNS Depressants (e.g., alcohol, opioids):

  • Increased risk of respiratory depression and sedation.

2. Antihistamines:

  • Additive sedative effects.

3.CYP450 Inhibitors (e.g., fluoxetine):

  • May prolong the action of lorazepam.

Contraindications

  1. Severe respiratory insufficiency.
  2. Acute narrow-angle glaucoma.
  3. Known hypersensitivity to benzodiazepines.
