Generic Name: Simvastatin
Common Brand Names: Zocor, Simlup, Simcard
Introduction
Simvastatin is a lipid-lowering medication belonging to the statin class. It is primarily used to reduce cholesterol levels and prevent cardiovascular diseases by inhibiting the synthesis of cholesterol in the liver.
Conditions of Use (Indications)
1. Hypercholesterolemia:
- Treatment of elevated total cholesterol, LDL cholesterol, and triglycerides.
2. Cardiovascular Disease Prevention:
- Primary and secondary prevention of atherosclerotic cardiovascular events in individuals with or at high risk for coronary artery disease.
3. Familial Hypercholesterolemia:
- Both heterozygous and homozygous forms.
Mechanism of Action
Simvastatin is a prodrug converted in the liver to its active form. It inhibits HMG-CoA reductase, the rate-limiting enzyme in cholesterol biosynthesis. This leads to decreased hepatic cholesterol production, upregulation of LDL receptors, and increased clearance of LDL cholesterol from the bloodstream.
Side Effects
1. Common Side Effects:
- Headache, gastrointestinal discomfort, fatigue, and myalgia.
2. Serious Side Effects:
- Rhabdomyolysis, myopathy, hepatotoxicity, and increased risk of diabetes mellitus.
Harmful Interactions
- CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin):
- Increased risk of myopathy and rhabdomyolysis.
- Fibrates (e.g., gemfibrozil):
- Additive risk of muscle toxicity.
- Warfarin:
- Potentiation of anticoagulant effects, increasing bleeding risk.
Contraindications
- Active liver disease or persistent unexplained elevation of liver enzymes.
- Pregnancy and lactation.
- Hypersensitivity to simvastatin or other statins.
